YOU ARE RESPONSIBLE FOR PROVIDING US WITH ACCURATE INFORMATION FOR YOUR PLAYERS!

TO ENSURE YOUR PLAYER REGISTRATION FORM AND PAYMENT ARE RECEIVED, PLEASE REGISTER AND PAY FOR EACH PLAYER SEPARATELY. 

Consent to Play

You MUST be the LEGAL parent or guardian to sign the below waiver, consent and register ANY player.

I represent that I am the parent or legal guardian of participant named below. I hereby acknowledge that Ivins Soccer is separate from, and is not a program of Ivins City. In consideration of Ivins Soccer accepting my child's entry and allowing my child to play soccer on Ivins City, Washington County School District fields or Washington County Parks, I hereby voluntarily and knowingly for myself, my heirs, executors, administrators and assigns waive and release, indemnify and hold harmless Ivins Soccer, Ivins City, The Ivins Parks & Recreation Department, Washington County School District, or Washington County Parks, as well as their agents and employees or anyone acting in consent with them for all harm, accidents, personal injury, property damage, or even death suffered by my child or myself and/or arising out of, or in any way connected with participations in the Ivins Soccer program, including all travel in connection with the said activity.

I understand that Ivins Soccer does not provide medical or accidental insurance and that I am responsible for all medical costs if my child is injured. I hereby certify that my child is in good health and capable of participating safely in the Ivins Soccer program and that my player has accident and health insurance.

As a legal parent/guardian of the above-named child, I hereby acknowledge that my child and I are aware that participating in soccer is a potentially hazardous activity. I hereby assume all risks associated with my child's participation in this sport, including but not limited to falls, contact with other participants, personal injury (bodily and/or emotional injury), death, the effects of the weather, traffic, and other risk conditions associated with this program. All such risks to my child are known and understood by me.

I hereby authorize the staff of Ivins Soccer to act in my behalf in accordance with their best judgment in case of an emergency. I authorize the Ivins Soccer staff to provide emergency treatment of an injury or illness of my child if qualified medical personnel consider treatment necessary and perform treatment. My authorization is granted only if I cannot be reached and a reasonable effort has been made to do so.

I understand that by registering for Ivins Soccer I agree to the Ivins Soccer refund policies which can be found under the FAQ section. I also understand and agree that any season cancelled PRIOR TO THE SEASON STARTING due to issues or complications related to Covid-19 or any other unforeseen circumstances, will result in my child being eligible to receive a partial refund and their individual uniform. I understand that once the season has begun, NO REFUNDS will be issued.

I understand and agree that I, my child, and all spectators involved with my child will abide by all rules of Ivins Soccer, Ivins City, as well as all local and state government rules and regulations. I understand and agree that I, my child and all spectators involved with my child will conduct ourselves in an appropriate and sportsman like manner. If we fail to do so, I understand that we will be asked to leave the facility where the game/practice is being held for the remainder of the game and possibly for the remainder of the season. If our behavior is of violent nature, I understand that the Police will be called and charges, if warranted, will be filed.

Ivins Soccer Code of Conduct

This Ivins Soccer code of conduct has been created to define the standards of ethical and moral behavior for all parents, players and spectators affiliated with Ivins Soccer.

1. I will emphasize that the game is played for fun and for the benefit of the youth.

2. I will act as a role model by demonstrating positive support for all players, coaches and game officials.

3. I will not engage in any kind of unsportsmanlike conduct with any official, coach, players, parent, or spectator such as booing, taunting, refusing to shake hands, or using profane language or gestures.

4. I will do my best to teach my child to play within the spirit and intentions of the rules and the Laws of the Games.

5. I will refrain from criticizing the game officials, and will respect their authority and decisions during the games.

6. I will NEVER place the value of winning over the safety and welfare of the youth.

7. I will demand a sports environment free from drugs, tobacco, and alcohol and will refrain from their use at all sporting events.

I understand that if I fail to abide by the Ivins Soccer code of conduct, I will be subject to:

 Verbal warning by an official, team coach, and/or Ivins Soccer
 Parental suspension with written documents to be kept on file with Ivins Soccer
 Game forfeiture through an official or Ivins Soccer

I have read this Ivins Soccer Code of Conduct. I understand its requirements and agree to abide by the aforementioned Ivins Soccer Code of Conduct.

PLEASE NOTE: ALL TEAMS ARE PUT TOGETHER BY SCHOOL (NOT NEIGHBORHOOD) AND BY DATE OF BIRTH (NOT GRADE). *** If you live in an outlying area (Diamond Valley, Little Valley, etc.) players will be placed in groups of 2-3 players on a team to prevent fully formed teams form entering the league. ***
